Prayer Book Dedication Form

Thank you for choosing to dedicate a prayer book in tribute to a loved one. 


Please complete the form below to submit your request, and allow two business weeks for your book plate to be made.

Sponsor a Shabbat Oneg

Please complete the following form to sponsor a Shabbat oneg. Your sponsorship includes: challah, juice, wine, Mediterranean platter, cheese and crackers, veggies and dip, linens, and flowers. 


Your submission must be received at least 5 days prior to your oneg date. Suggested minimum donation of $540.00 for a standard oneg or $720.00 to include a cake. Donation values represent suggested amounts; please choose an amount that is comfortable and meaningful for your family. 



Please note, If your date is shared with another member,  the temple will recognize each family accordingly. If you are a parent of a b'mitzvah student, your oneg is already included in your b'mitzvah fee and you do not need to complete this form.

Yahrzeit Memorial Plaque Order Form​


The anniversary of the death of a loved one is naturally a solemn day, and our custom of a Yahrzeit memorial helps us remember and honor the memory of our beloved ones. 


Plaques include a ledge for a stone to be placed beside your loved one's name on the anniversary of their passing and when yizkor is observed on Yom Kippur, Shemini Atzeret, Simchat Torah, the seventh day of Pesach and Shavuot. 






It is understood that each year on the yahrzeit of the departed, and on other appropriate occasions in keeping with Jewish custom, the name of the departed will be read during the Shabbat service and a memorial prayer will be recited. Small stones are available for visitors to place at the nameplate as a sign of remembrance.
